Certificate of Completion
After completing the 4-week ARP, facilitators can download and print off this certificate of completion. We suggest you purchase a package of certificates with borders to save ink.
Program Evaluation Form
Participant feedback is valuable for program improvement and promotions, so please use this form during your last session. The format of this form is two evaluations per sheet to reduce printing costs.
